SSS Branches With SSS ID Enrollment Facilities in Metro Manila and Rizal

Here's a list of SSS branches in Metro Manila and Rizal where there are SSS ID enrollment facilities:

Para maka-apply for an SSS ID, dapat active member ka at meron ka nang na-post na contribution.
You must have at least one monthly SSS contribution that has been posted in your SSS record.

Sometimes, the ID capture machines are out of order, so if you're busy, it is best to call first before going to the branch.

Bring 2 valid IDs. Fill up the SSS Form E-6. Write your permanent address in the form, not your temporary address, because ID processing might take a long time. Your SSS ID will be mailed to your address.

When making follow-ups, present your IDs and acknowledgement slip (this will be given to you after your application is processed).

 The SSS ID is free for first-time applicants.

What are the requirements if I have to chnage the Gender on my SSS? It appears that my gender in SSS is "MALE" where in fact i am FEMALE, I am now worrying because I will give birth this year and I intend to file for Maternity Benefits. Thanks

Hi Winter, file E4 form with SSS -- merong Correction of Gender in this form. Wala namang nakasulat na requirement; pero dalhin mo lang ang birth cert mo.

Hi Deanne, bring your marriage certificate (original and xerox) and fill up E4 form. About SSS ID application, go to a branch with a photo capturing system with your other valid IDs and fill up E6 form. Please read above post for more info.

Hi kagetwentyseven, yes, based on the SSS instruction that we should transact at the branch nearest our place of work or residence.

Hi Fancy, bring your ID and your child's birth certificate, if for your child, and marriage cert, if for your spouse (xerox and orig); your birth cert, if for your parents. Fill up E4 form

Hi,

I checked my online actual premium and I found out that their are no contribution for july and august 2013. I am still with the same company. My payslip show they deducted my share for sss. I wanted to know what is the right process on handling this case, cause I got another co worker and she have 1 year no contribution and up to know the issue is still not solve. I want to know what is the fastest means to resolve the case

Hi SKYLER, ask your HR to correct your SSS posting. Print out the online page showing your contributions, and then encircle July and Aug 2013 with pen. Attach a xerox of your Jul and Aug 2013 payslips.
The process is for your HR to find the collection list and OR for July and Aug 2013, xerox them, and then file with SSS together with a written request for correction.
About your co-worker with 1 year of unposted contribution, that's so bad. Advice him/her to do what I advised, with 1 addition -- a written request to your HR to fix his/her SSS remittances. Xerox the written request and ask the HR to sign his/her copy (for future actions, if request is not done).

Hello, i need help. I need to apply for my SSS ID but they didn't allow me because my payment was not posted. They referred me to DOC Encoding for manual encoding, and they said this will take 2 weeks. But that was last July. Where can I ask for help to apply for my ID. By the way I'm self-employed.

Hi Remil, did you use RS1 form when you registered as self-employed? Was this form approved by SSS before you paid your first contribution? If yes, then continue paying for Aug and Sep and wait for posting of your Date of Coverage (DOC). DOC posting might take 3 months or more, based on comments online.
